These labels have the incorrect Ratio and Plant Part printed. The ratio should be printed as 1:4 and the Plant Part should be printed as Dry Root. All other information is correct.

After receiving this material in Unit 2 from Dogwood, (x3) bags were noticed to be dirty and have rust stains.

3 boxes of Holy Basil Fresh OG arrived at Madrone from the vendor Kohnert Organic Farms. Upon further inspection it appears that the material was cut at the base of the stalk, which leaves a lot of material that is not used for making this extract. Additionally, it was noticed upon arrival that one box was not taped/ sealed properly leaving an opening exposing a small amount of product.

Due to metal bag liner in packaging, this material must be repacked.

We are discontinuing this offering and noticing the age of the RM in stock, we can get rid of it and sell out the remaining FG material per Products Department.

511.95 Lb. of Witch Hazel Bark Rough Cut NOG Lot# H01084-004 were brought to the mill to be processed in the cutting room.

It appears that 2 different RM lots of this material was used to produce 20lb FG 2609-00057.

The forks of a pallet jack punctured the bag.

Rainwater slightly flooded the Quonset area reaching the exterior of the Mill's north wall. There was minor water seepage into the mill's interior. The facility personnel has already cleaned and sanitized the affected area. QC Mill will perform an unplanned environmental swabbing. Pigs sorbent socks were used to contain the flood.

While putting away partial bags, Riley found 1 bag @ 35.2 lbs with a tear in location at 50-DOG-T2.

It was decided at HCI that 3 bags (132 lbs) from the bags that were already sampled is to be unrejcted for remediation for excreta.

Water from the A/C Unit leaked through the ceiling. . Water seeped through the clamp that holds the fire suppression pipe and dripped onto the chair located in a the left corner of the back grinding room, away from production area. The water leak did not affect the product or the grinding process. Back Grinding personnel is going to clean and sanitize the affected area and QC Mill will perform an environmental swabbing. The production is halted until further notice.

HVAC unit has been having drainage issues and we need to determine any risk to product

1lb of Garlic Salt OG RM lot# 2602-M0068 was decanted into a new 56.96lb box of Garlic Salt OG RM lot# 2604-M0128 prior to being taken to a production station.

This room and equipment was used to powder Anise Seed H00021-004. The room and the equipment need environmental swabbing to determine no contamination.

On 08-21-2026 - Metal Remediation personnel found a live larva while processing the product.
2120 Lbs. (42@ 50 Lbs., 1@ 20 Lbs.) were scheduled for metal inspection; during the process a live larva was observed in the product. QC Mill inspected the 43 boxes and I was instructed to conduct a high risk EMI; no further live larvae were observed during either inspection, but rodent droppings were found. The metal detection process was stopped and the two pallets were wrapped in plastic. QC hold sing was placed on both pallets.

Due to this material (1,014 lbs) being received in supersacks (1 @ 196, 200, 231, 201 and 186) it could not be sampled for testing.

According to the back grinding personnel some cigarette butts were found in the remaining pounds of Lemongrass C/S. QC Hold was placed on the product. The process was stopped pending further disposition.

A piece (1@ 24mg, 8mm in length) of rodent excreta was observed and collected during the remediation process.
The remediation work continues with 18 unprocessed 55 pounds bag.

This matcha set is a different design and would expect the lot number (UPC code) to also be different then our other stock Per CR.

Upon visual inspection, boxes were observed with various tears/punctures and crushing damage. These boxes were numbered to aid in identification. Box #3 @ 44 lbs had external and internal damage to the box and one of its bags measuring 70mm x 55mm. Box #5 @ 44 lbs was completely impaled. Damage penetrated through both inner bags as well as both sides of the box.

Initial testing was over spec for peroxide value. (15.7 meq/kg)

M300 Grinder#3 metal screen broke during the grinding process. The broken metal screen affected only (01@ 32.60 Lbs.) box of processed product. QC hold sign was placed on the box.
A new, clean metal screen was installed so production can continue on the remaining pounds.
(03@ 44 Lbs.) bags were brought to Trillium Mill for grinding.
On 07-20-26, A second metal screen broke possibly affecting 3.10 Lbs. of product. QC Hold sign was placed on the box. LAT.

While powdering this material a piece of glass was found.

On this Fresh Maceration the two Vats that were used did not have the correct number of additions applied to the Vats. This resulted in the Yield being low at 76% and it will also make the ABV off as well.

This mateial was received from Trout Lake Farm LLC and originaly rejected. It was decided that this material is to be remediated instead per Meghan B.

It was discovered that some labels printed on the Mark Andy printer for products at Madrone were missing the servings per container information.
