We received in 160 @ Fire Cider Classic 8oz labels. The Label request was for 16oz labels. When scanned these labels come up as 16 oz, but the Labels themselves physically have 8oz on them.

We received 216 @ Rose Hydrosol 16oz (Lot # 2607-00786) labels on 2 rolls. 1 of the 2 rolls has an error code when scanned that comes up "Hello World". This affected 106 of the 216 labels that we received.

We received in 530 @ 205 @ Rose Hydrosol OG 8oz Labels (Lot # 2607-00785) that were split into two rolls. When QC checked these labels 1 roll that had 270 labels on them were coming up with "Hello World" when the Lot number was scanned.

This is a bulk return we were holding on too hoping to sell it again. Now the RM is out of stock and this can be used for blending.

We received 205 @ Rosemary Hydrosol OG 8oz Labels and when doing a QC Label content check, it was found that every other label when scanned was coming up with " Hello World" instead of the Lot number.

Upon inspection it was observed that a parial bag of 14.3# appeared to be only fines and overs and was inconsistent with the appearance of the rest of the product.

It was noticed by the decanter in Unit 5 that a box has 2 MRH labels on it, One that says Genmaicha Tea T00056-003 and one that says Spring Green Tea T41822-001.

It was noticed by Jon H. in Dry Production that the (x275) Coarse Himalayan Pink Salt NOG 1lb labels have the incorrect size printed.

It was noticed by the decanter in Unit 5 that a box has 2 MRH labels on it, One that says Genmaicha Tea T00056-003 and one that says Spring Green Tea T41822-001.

One shattered Amber Glass Bottle 8oz was found in a flat of 54 each of Lot # C00044-006. 22 @ 8oz Amber Glass bottles were produced prior to the filler noticing the broken glass bottle in the flat.

It is stated by the Blender on the Dry Ingredient Blending Batch Record that the batches still clumpy after blending, will require further processing at the mill.

Vendor pallet broke during restacking; 6 cases @ 108 ea fell off the back and contacted the ground; possible broken glass.

While restacking the vendor pallet, 1 bag @ 44 lbs was found to have pallet nail puncture holes.

While restacking a pallet, the string closure of 1 bag @ 55 lbs was caught on another bag and pulled out. Material spilled out of the top of the bag (loss of weight).

While picking, 1 bag @ 39.6 lbs was found to have a puncture in the front of the bag, allowing some product to spill (loss of weight).

M300 Grinder#3 metal screen broke during the grinding process. The broken metal screen affected only (01@ 32.60 Lbs.) box of processed product. QC hold sign was placed on the box.
A new, clean metal screen was installed so production can continue on the remaining pounds.
(03@ 44 Lbs.) bags were brought to Trillium Mill for grinding.
On 07-20-26, A second metal screen broke possibly affecting 3.10 Lbs. of product. QC Hold sign was placed on the box. LAT.

Due to this material (800 lbs) being received in supersacks (180 lbs, 221 lbs, 200 lbs, & 199 lbs) it could not be sampled for testing.

This material was returned by a customer due to "did not pass their Gluten test". They returned 50lbs of this finished goods lot.

Prior to production the material was noticed to have solidified into one large clump of herb.

On this Fresh Maceration the two Vats that were used did not have the correct number of additions applied to the Vats. This resulted in the Yield being low at 76% and it will also make the ABV off as well.

This mateial was received from Trout Lake Farm LLC and originaly rejected. It was decided that this material is to be remediated instead per Meghan B.

Live cigar beetles observed within product during inspection; approximately 15 insects. No samples obtained at this time. Pallet must be wrapped and isolated to await disposition.

During the final QC check blue soft plastic and what appears to be clear polywoven strands were noticed in some bags.
