Upon further inspection it appears that the material was cut at the base of the stalk, which leaves a lot of material that is not used for making this extract. Josh C. had discovered (x12) bags of Elder Flowers Whole OG 4oz with incorrect FG lot #'s (2605-M0005) inside of a box. The correct FG lot # is 2605-M0004.

It was decided at HCI that 3 bags (132 lbs) from the bags that were already sampled is to be unrejcted for remediation for excreta.

Water from the A/C Unit leaked through the ceiling. . Water seeped through the clamp that holds the fire suppression pipe and dripped onto the chair located in a the left corner of the back grinding room, away from production area. The water leak did not affect the product or the grinding process. Back Grinding personnel is going to clean and sanitize the affected area and QC Mill will perform an environmental swabbing. The production is halted until further notice.

This was received with incorrect Country of India on vendor's RMC. Correct Country of Origin is Nepal.

While performing a QI Fill Check on 8/28/26, I had noticed that the QI Fill Check BoilerPlate from 8/27/26 was not completed for the FG that was produced on that day.

575 @ Golden Milk Tea OG Tins 5.2oz labels were requested and supposedly 586 were printed, but these labels cannot be found.

When brought into the warehouse, 2,200# of this material was observed to have approximately 10 living fruit flies on exterior of the bags per pallet (2 pallets total), within the pallet wrap. Unconfirmed at this time if insect activity exists within these bags. No other insect activity observed in the vicinity of this product. Inventory crew is wrapping up and isolating the 2 pallets of this product to await disposition.

While picking this item, 1 box @ 240 ea was found to have broken glass inside.

On 08-21-2026 - Metal Remediation personnel found a live larva while processing the product.
2120 Lbs. (42@ 50 Lbs., 1@ 20 Lbs.) were scheduled for metal inspection; during the process a live larva was observed in the product. QC Mill inspected the 43 boxes and I was instructed to conduct a high risk EMI; no further live larvae were observed during either inspection, but rodent droppings were found. The metal detection process was stopped and the two pallets were wrapped in plastic. QC hold sing was placed on both pallets.

Live cigar beetles observed on exterior (approximately 50 - 80) and interior (3 within 1 bag) of this material on 1 pallet from 50-DOG-J11. No cigar beetles observed on material from 50-DOG-N83 or 50-DOG-R67. No insect activity observed adjacent to the mentioned storage locations. Inventory crew alerted to wrap the 3 pallets and move them to a quarantine area.

We received 50 @ 22 bags from Peruvian Nature without inner liners. Most of the bags appear to have a varying quantity of small puncture holes measuring 10mm in diameter, at all varying locations of the bags. Punctures were not repaired; products are contained within loose polyweave bags with what appears to be product (NOG) dust on their exteriors.

Upon visual inspection, Box #1 @ 55 lbs had a tear measuring approximately 250mm x 245mm to the outer box and torn inner plastic that could not be measured at the time without causing further damage and spillage.

We received 216 @ Rose Hydrosol 16oz (Lot # 2607-00786) labels on 2 rolls. 1 of the 2 rolls has an error code when scanned that comes up "Hello World". This affected 106 of the 216 labels that we received.

We received in 530 @ 205 @ Rose Hydrosol OG 8oz Labels (Lot # 2607-00785) that were split into two rolls. When QC checked these labels 1 roll that had 270 labels on them were coming up with "Hello World" when the Lot number was scanned.

This is a bulk return we were holding on too hoping to sell it again. Now the RM is out of stock and this can be used for blending.

We received 205 @ Rosemary Hydrosol OG 8oz Labels and when doing a QC Label content check, it was found that every other label when scanned was coming up with " Hello World" instead of the Lot number.

Upon inspection it was observed that a parial bag of 14.3# appeared to be only fines and overs and was inconsistent with the appearance of the rest of the product.

While restacking for put away, 1 bag @ 35.2 lbs was found to have a puncture hole underneath the vendor label that says "Punto di prelievo"

Upon visual inspection 12 boxes were confirmed to have sustained numerous tears, punctures and crushing. Of these, 9 boxes contained tins with visible denting. The 9 boxes were the following:

During the production of 2605-00941, what appears to be Wild Yam Root has possible mold.

M300 Grinder#3 metal screen broke during the grinding process. The broken metal screen affected only (01@ 32.60 Lbs.) box of processed product. QC hold sign was placed on the box.
A new, clean metal screen was installed so production can continue on the remaining pounds.
(03@ 44 Lbs.) bags were brought to Trillium Mill for grinding.
On 07-20-26, A second metal screen broke possibly affecting 3.10 Lbs. of product. QC Hold sign was placed on the box. LAT.

Quarterly testing has come up for our water system. The PEX lines were also replaced. The lines were flushed with a few gallons of water prior to obtaining samples.

There is a 5 Gallon bucket decanter for Rose Hydrosol OG Lot# B00013-006 that was made on 1-12-26 that has suspected Mold growth inside.
