During production (2610-00575) the decanter brought a RM bag and when opened it appears to be solidified into a block.

This material was received in burlap sacks (60 @ 55 lbs).

QC sampler reported on the receiving log that the exterior of the bags have product dust on surface of bags.

Jarrett was receiving PO5469 (GRPO 5176) from Young Mountain Tea and accidentally received the amount as "386.8 lbs".

It was discovered that Unit 4 was only sent a total of (x557) Yerba Mate OG FT 1lb labels instead of the (x900) that was printed.

When Scott had finished pouring the 8oz bottles of Gotu Kola Extract OG, he noticed a few dead gnats at the bottom of pitcher he was using to fill the bottles.

When material came back from treatment, moisture content was slightly elevated. Material was put on IH to determine if issue would persist over 3 months or moisture content would decrease naturally.

During the milling process , the personnel made observations and collected pieces of rubber bands, soft plastic. and a small metal fragment recovered from the magnet. The sample of foreign matter that has been obtained will be delivered to HCI Room.

During the production of FG 2609-00379 the herb filler found what appears to be (X1) down feather.

When receiving this product back from 3rd party processor Mueggenburg, it was mistakenly given a '-T' for steam treatment instead of the correct '-P' for powdered material.

Oregon Black Tea was received in the amount of 5 lbs in Inventory to match the PO, but the individual bag weights add up to 5.46 lbs.

While attempting to obtain a sample for a 2 year retest, approximately 50 - 100 live cigar beetles were observed within the inner bag of 1 container. Inventory crew alerted

It is stated by the Blender on the Dry Ingredient Blending Batch Record that they recommend this to be sent to the mill for further processing/sifting.

Failed to Powder: 810 Lbs. (54@ 15 Lbs.) of Cat's Claw Bark C/S NOG were brought to be ground. After several attempts using the M300 Grinder#1 and the Fitz Mill equipment, the grinding process had to be cancelled. The product is very difficult to process, very hard to grind. Only one pound was wasted from a 15 Lbs. bag.

This was a preship sample from Onset. We tested it at Element Labs and got a result that was elevated for peroxide value (10.5 meq/kg). The vendor provided a conflicting result from their 3rd party lab (1.352 meq/kg).

These 23 @ Gotu Kola Extract OG 32oz labels have an extra 2 at the beginning of the FG Lot number, 22610-M0037. These labels scan correctly; it's just the physical labels themselves have that extra digit at the start of the Lot number.

While performing a label content check for Saw Palmetto Extract OG 8oz, I noticed the plant part is listed as "Fruit", the correct plant part is "Dried Fruit".

While offloading a pallet of Sweet Cinnamon Chips OG, receiving noticed 1 bag @ 13.2 lbs of Witch Hazel Leaf C/S NOG- H01085-029-T was on top of this pallet. This bag of Witch Hazel appeared to be damaged and spilling onto the bags of Sweet Cinnamon Chips OG.

Upon visual inspection 1 bag @ 13.2 lbs was confirmed to have damage in the form of a tear.

Jordan was using the Dose it and pouring some Lemongrass E.O. OG (Lot # ES00063-011), after he filled the first (2) 40 Fl oz measuring cups, while filling the 3rd, the material started to turn a much darker color then what we are used to seeing. This is when he stopped and notified QC.

This material is kept in a hard cardboard cylinder container that has a metal clamp/seal. The metal part of the seal is broken and appears unsafe.

A variety of nut snack packs was ordered by the store director for sale in the mercantile locations. These represent an allergen hazard and are not currently documented on our allergen plan. This did not go through formal product or process approval steps.

While I was performing an ATP swab for Logical 2 in unit 4, I had mistakenly tested under the "Tea Blending" site.

During the production of Sweet Cinnamon Sticks lot #2609-M0211 an unknown white substance was found on on some of the material.

(x3) 25lb bags were received from Dogwood, (x1) bag has 2 different lot #'s on it.

There was a single ingredient in this Crone Care Combo that was over issued when this batch was receipted. 9.60L of Motherwort Extract OG (Lot # 2505-M0214) was accidentally issued instead of what the recipe calls for, which is 7.60L. This resulted in 2 L extra of Motherwort Extract being issued to this Crone Care Extract batch.

We received a customer complaint for 30lb 2608-00675 for Y/M results exceeding their spec.

(x1) 16oz bottle with 2 of the same labels was found while stocking the pick floor.

During QI FG check, a small piece of what appears to be green electric wire coating was found. It measured 15 mm long and 2 mm wide.

There was a customer complaint called in for live insects in a 1 # Linden C/S Lot # 2606-M0199. That bag used the same Raw Material, RM Lot # H00334-074-T that the 4oz bags were filled with.

On 9/28/26 QC received a customer complaint regarding live insects in their 1 # pound bag of Linden.

It is noted by Samplers that the vendor labels on these bags have a "USDA Organic" marker on the bottom right corner. This material is not organic.

They received (x4) labels for Tamanu Oil OG 5gal, the FG lot# (2609-00503) scans as Refined Sweet Almond Oil OG 5gal. Upon further investigation, these labels were requested to be printed as Refined Sweet Almond Oil 5gal 2609-00503.

The label request form indicates that 80 labels were printed, but only 7 were received on the roll.

(x1) of these cello bags containing Cotton Muslin Bag Large (case of 100) has split open.

While Jason was pulling out the pallet at 50-DOG-E4 (one of the tighter aisles), a bottom bag (1 @ 31.9 lbs) tore against the rack support. Torn bag was placed into a polywoven bag to contain product.

While transferring product onto a plastic pallet for Unit 4, 1 bag @ 44 lbs became caught on the plastic pallet and peeled back a section of the bag. Approximately 2 lbs was lost to spillage (please reweigh).

It was noticed by Jon H. in Dry Production that the labels for 1# Honeyroo Tea OG (2609-00152) does not have Red Rooibos Tea listed as Fair Trade in the ingredients list.

A piece of green rigid plastic that measured 6x6 mm was found during production. This measurement is not a choking hazard.

While picking for the run, Jason noticed that 1 box @ 360 ea sounded like it had broken glass in it.

While stocking the pick floor with Cacao Nibs Roasted C/S OG FT 1lb 2602-00383, a bag labeled 4oz Dandelion Leaf 2602-00385 but filled with Cacao Nibs Roasted C/S OG FT was found.

3 boxes of Holy Basil Fresh OG arrived at Madrone from the vendor Kohnert Organic Farms. Upon further inspection it appears that the material was cut at the base of the stalk, which leaves a lot of material that is not used for making this extract. Additionally, it was noticed upon arrival that one box was not taped/ sealed properly leaving an opening exposing a small amount of product.

Due to metal bag liner in packaging, this material must be repacked.

We are discontinuing these per Products Department.

Josh C. had discovered (x12) bags of Elder Flowers Whole OG 4oz with incorrect FG lot #'s (2605-M0005) inside of a box. The correct FG lot # is 2605-M0004.

Extraneous matter was observed during processing. According to the back grinding personnel some cigarette butts were found in the remaining pounds of Lemongrass C/S. The personnel also recovered small clumps of dry soil, small piece of plastic with metal and other foreign matter.

While relabeling this product with 'CLEARANCE' labels, 1 bag @ 48.4 lbs was found to have small tears at each end of the bag (possibly from it's position on the pallet/rubbing, etc).

This material contained an ingredient that was used in the Nurse-Me Rhyme Tea OG (2606-M0260) which was out of specification.

On the pick floor, (x3) bags filled with Moringa Leaf C/S OG 2606-00262 were found with Catnip C/S OG 2606-00219 labels.

On 8/31/26, CAH had input their initials into a filled by BoilerPlate but did not produce any material that day.

Extraneous matter was found during the production of 1# Bilberry Fruit Whole OG (2608-00301).

While putting away partial bags, Riley found 1 bag @ 35.2 lbs with a tear in location at 50-DOG-T2.

This material is now a clearance lot and should be labeled as such.

It was decided at HCI that 3 bags (132 lbs) from the bags that were already sampled is to be unrejcted for remediation for excreta.

Water from the A/C Unit leaked through the ceiling. . Water seeped through the clamp that holds the fire suppression pipe and dripped onto the chair located in a the left corner of the back grinding room, away from production area. The water leak did not affect the product or the grinding process. Back Grinding personnel is going to clean and sanitize the affected area and QC Mill will perform an environmental swabbing. While performing a QI Fill Check on 8/28/26, I had noticed that the QI Fill Check BoilerPlate from 8/27/26 was not completed for the FG that was produced on that day.

575 @ Golden Milk Tea OG Tins 5.2oz labels were requested and supposedly 586 were printed, but these labels cannot be found.

When brought into the warehouse, 2,200# of this material was observed to have approximately 10 living fruit flies on exterior of the bags per pallet (2 pallets total), within the pallet wrap. Unconfirmed at this time if insect activity exists within these bags. No other insect activity observed in the vicinity of this product. Inventory crew is wrapping up and isolating the 2 pallets of this product to await disposition.

2120 Lbs. (42@ 50 Lbs., 1@ 20 Lbs.) were scheduled for metal inspection; during the process a live larva was observed in the product. QC Mill inspected the 43 boxes and I was instructed to conduct a high risk EMI; no further live larvae were observed during either inspection, but rodent droppings were found. The metal detection process was stopped and the two pallets were wrapped in plastic. QC hold sing was placed on both pallets.

Live cigar beetles observed on exterior (approximately 50 - 80) and interior (3 within 1 bag) of this material on 1 pallet from 50-DOG-J11. No cigar beetles observed on material from 50-DOG-N83 or 50-DOG-R67. No insect activity observed adjacent to the mentioned storage locations. Inventory crew alerted to wrap the 3 pallets and move them to a quarantine area.

We received in 530 @ 205 @ Rose Hydrosol OG 8oz Labels (Lot # 2607-00785) that were split into two rolls. When QC checked these labels 1 roll that had 270 labels on them were coming up with "Hello World" when the Lot number was scanned.

This is a bulk return we were holding on too hoping to sell it again. Now the RM is out of stock and this can be used for blending.

We received 205 @ Rosemary Hydrosol OG 8oz Labels and when doing a QC Label content check, it was found that every other label when scanned was coming up with " Hello World" instead of the Lot number.
